About The Position

On-site Director of Patient Care Services (DPCS) is responsible for the overall direction of home health clinical services. The Director of Patient Care Services establishes, implements and evaluates goals and objectives for home health care services that meet and promote the standards of quality and contribute to the total organization and philosophy. Your responsibilities will include: You will be the oversight manager for some field clinical staff and some office staff. Reporting to the Administrator, you will be responsible for the implementation, delivery, coordination, with managerial oversight, of the clinical and administrative home health services delivered by your team consistent with best practices, agency policy and procedures. You participate in and provide leadership for the team during regular clinical meetings. You routinely review and revise the patient/family care plans. You serve as a member of the clinical leadership team addressing problems associated with the delivery of clinical services, identifying staff development needs, and suggesting appropriate changes in policies and procedures. You supervise the maintenance of medical records ensuring accuracy, completeness and compliance with regulations, legal and ethical standards and that the team meets compliance standards on quarterly and random clinical record audits. You collect and review pertinent clinical data at team meetings. You submit Plans of Correction to the Administrator when required by audit results and/or patient family satisfaction surveys. You monitor documentation of all team members and advise when documents do not meet company standards. You conduct supervisory visits to observe the performance of all team members. You provide several customer in-services to each contracted facility required by law and provide appropriate supporting documentation. You arrange team coverage for on-call and staff time off. You provide leadership and counsel to enhance staff development. You meet budgeted Average Daily Census and Admissions goals. You meet reasonable referral conversion goals as set by the Administrator. You engage regional community liaisons regarding business development and display a commitment to the growth of the region and organization.

Requirements

  • Significant prior Home Health RN supervisory experience is a must.
  • Must have DPCS experience pf 3+years.
  • Current or renewable RN license is mandatory.
  • Must have a valid Driver’s license and reliable transportation.
  • Strong verbal and written communication skills.

Nice To Haves

  • BSN is preferred for Home Health DPCS, or an ASN with hand-on clinical experience.
  • 3+ years previous experience as a Home Health RN is much preferred.
