Requirements
- Active and unrestricted RN License in IL
- Minimum 2 years of home healthcare experience (required)
- Minimum 2 years of nurse management or leadership experience in home health (required)
- Strong understanding of Medicare, Medicaid, OASIS, PPS , and home care operations
- Knowledge of state and federal home health regulations
- Excellent computer and documentation skills
- Strong leadership, problem-solving, and communication skills
Responsibilities
- Direct and manage all office clinical operations, including patient assessments, care planning, and issue resolution.
- Ensure compliance with company policies, state/federal regulations, and payer requirements.
- Maintain quality and adequacy of nursing practices and patient care services.
- Oversee communication among clinical staff, patients, families, and referral sources.
- Participate in agency performance improvement and community outreach initiatives.
- Provide backup support for field nurses as needed (SOC, ROC, discharge visits, etc.).
- Lead, coach, and mentor clinical staff to promote excellence in care and teamwork.
