About The Position

Join Lifeworks as a Direct Support Professional 2 and make a meaningful impact every day. We are seeking a full-time DSP 2 to support adults with disabilities in New Hope, the surrounding community, and virtually. In this role, you will build strong relationships and empower individuals through engaging, goal-focused activities that help them achieve their long-term aspirations. You will manage a caseload, attend intakes and annual meetings, complete required documentation, and collaborate closely with individuals and their support teams to ensure person-centered planning and services.

Requirements

  • Four-year degree in human services or a related field; or a two-year degree in Human services or related field and two years of work experience; or a combination of education and experience to equal four years.
  • Experience working with people with disabilities.
  • Access to reliable transportation for work driving purposes.
  • Valid driver’s license and proof of auto insurance.
  • Candidate must complete and pass a pre-employment background study; a physical and drug screen; and have access to reliable transportation for work purposes.

Nice To Haves

  • Experience managing a caseload, coordinating with support teams, writing goals, and person-centered planning preferred.

Responsibilities

  • Work closely with program team to meet the daily needs of individual served
  • Identify, write, and implement new person-centered plans and goals for the individuals.
  • Effectively communication and coordinate with individuals’ support teams.
  • Organize group activities in the community or facilities based on the interests of the members of the group.
  • Transport individuals out into the community for volunteering and other outings using a company vehicle.
  • Ensure that individuals’ health and safety is maintained by following their individual plan of care and Lifeworks’ policies and procedures.
