Summary: Serves as Diabetes and Nutrition Educator for Diabetes and Nutrition Center through planning, implementing, and evaluating the delivery and quality of education provided to both health center patients, frontline staff, and the broader community. Serves as a mentor, role model, consultant, facilitator, and educator.

  • Registered Nurse Compact License (RN LICENSE COMPACT) - Compact RN Multistate, Registered Nurse License (RN LICENSE) - State of Florida (FL)
  • Complexity of Work: Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Excellent verbal and written communication skills.
  • Two (2) years' relevant work experience pertinent to Diabetes Self- Management Education and Support Training (DSMEST).
  • Experience with program planning and evaluation (i.e., pre- and post-test, repeat demonstration, clinical measures, evaluating trends, and communicating plan of care).
  • Additional Education Info: Dietetics or Nutrition or graduate of an accredited professional nursing program
  • Additional Credential Info: Registered Nurse License OR Registered Dietitian CDR Certification OR Registered Pharmacist; Certified Diabetes Educator OR Board- Certified Advance Diabetes Management (BC-ADM) certification preferred or completion of either Certificate within 12 months of hire.

Responsibilities

  • Collaborates with the healthcare team to integrate nutrition therapy into the management of diabetic patients' care.
  • Supports initiatives to decrease avoidable hospital utilization
  • Develops and provides individualized diabetes and nutrition education to patients and their families and caregivers.
  • Develops and implements educational activities for staff and the community.
  • Collaborates with key stakeholders to address social determinants of health and strengthen integration and linkages by wrapping community health programs and services around vulnerable populations.
  • Assesses patient understanding and cooperation with integrating nutritional planning into the patient's lifestyle. Modifies plans to promote success.
  • Participates in the required activities to maintain ADA Certification.
  • Serves as a resource for front line staff and supports education deficits.
