The DRN role is a non-24/7 nursing role, that crosses all continuum of health care sites functioning in the role of expert clinician, educator, and nurse care manager for diabetes patients across the continuum of care. In collaboration with the medical and ancillary staff, monitors the clinical care of patients and provides clinical support to improve patient care and outcomes, for both inpatient, outpatient, and across the continuum of care. Provides a significant level of disease-specific education to patient and hospital staff. Participates in assessing, planning, implementing, and evaluating health services of a disease specific population during hospitalization, post discharge, and site of service across the continuum. Performs tasks requiring independent knowledge and judgment. Implements performance improvement initiatives. Provides community outreach activities. Serves as a liaison and resource among patients, families, healthcare providers, staff, and community to provide a seamless transition throughout phases of care. Collects data for diabetes quality improvement initiatives and participates in implementation and management of performance improvement initiatives. Responsible for other Diabetes Services activities and assignments per Diabetes Services leadership as other duties as assigned.
