Registered Nurse Diabetes Prevention: Health Assessment and Promotion (Part-time) Mission: To provide outreach and direct services to support identifying and controlling cardiometabolic syndrome, including diabetes, hyperlipidemia, and hypertension, among underserved or high-risk populations.

Responsibilities

• Provide client management (blood pressure monitoring, control support, education, etc.) for persons diagnosed with primary hypertension.
• Provide management, treatment, control, and monitoring of hyperlipidemia per hypertension protocol guidelines.
• Client management (glucose monitoring, control support, education, etc.) for persons with the primary diagnosis of type 2 diabetes.
• Provide management, treatment, control, and monitoring of hyperlipidemia per type 2 diabetes protocol guidelines.
• Delivery of the Diabetes Prevention Program (DPP) for persons at risk of diabetes.
• Enroll a minimum of 15 participants per year (“Enroll” is defined as having a completed registration form and the individual attending at least 2 sessions).
• Support community outreach events: recruitment of patients, conduct screening and provide health education and promotion activities.
