About The Position

Provides group and individual Diabetes Education. Provides nutritional counseling to clients referred by a provider. Develops educational goals, learning objectives and a plan for educational content and teaching methods. Develops an individualized follow-up plan with each program participant. Requirements

Associates Degree Registered Nurse Required
2 years Relevant experience.
Licensed Registered Nurse Upon Hire Required or Registered Nurse Multi State License Upon Hire Required or Licensed Dietitian-Nutritionist Upon Hire Required
Heartsaver CPR/AED Upon Hire Required
Able to lead and manage groups.

Responsibilities

Provides group education and individual education in Diabetes Education.
Provides nutritional counseling to clients referred by a provider.
Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with program participants.
Manages blood glucose for patients.
Manages insulin pumps and continuous glucose monitors for patients.
Collects program participant assessment data, using the ADDE7 framework, in a collaborative and ongoing manner.
Collaboratively develops an individualized follow-up plan with each program participant.
Conducts a follow-up assessment upon completion of program services, using outcome measures from the AADE7 Continuum of Outcomes Framework.
Documents assessment data, educational plan, educational services provided and evaluation results in each participant's educational records.
Utilizes a team approach to provide services and collaborates and communicates with team members when needed.
Participates in peer review process to evaluate performance of other professional staff.
In-depth knowledge about current diabetes treatment management.
Ability to develop a collaborative, therapeutic alliance with individuals.
Provides outstanding customer service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.
Enhances professional growth and development through participation in educational programs, current literature, conferences, and workshops.
