Developmental Health Services Treatment Coordinator
About The Position

Join our team in Elmira, NY as a DHS Treatment Coordinator , where you'll play a key role in supporting individuals through coordinated care, treatment planning, and collaboration with multidisciplinary teams. In this role, you'll lead the coordination of clinical services, treatment plans, and intake processes , ensuring individuals receive high-quality, person-centered care. You'll work closely with Medical Directors, Care Managers, families, and service providers to deliver effective and compliant services.
Requirements
- Bachelor's degree in Psychology, Social Work, or related field
- At least 1 year of experience in clinical intake, case management, or healthcare coordination
- Strong communication, organization, and documentation skills
Nice To Haves
- Experience with Microsoft Office (Excel, Word, Outlook, PowerPoint)
- Familiarity with electronic health records (EHR) and clinical documentation
Responsibilities
- Coordinate clinical treatment plans, services, and follow-up care
- Conduct intakes, admissions, and obtain required authorizations
- Serve as liaison between providers, Care Managers, and families
- Supervise and support assigned staff
- Ensure accurate documentation, audits, and HIPAA compliance
