The Deputy Director of Care Transformation sits within the Agency of Human Services (AHS) Office of Health Care Reform (HCR) and is responsible for supporting activities related to care transformation, including identifying, developing, and implementing policy and programmatic changes in response to Act 167 and Act 68 to ensure financial sustainability and adequate access across the health system in Vermont. A portion of the role will include overseeing Rural Health Transformation Program (RHTP) activities related to Care Transformation and related day-to-day operations, grants and contracts management, and coordinating with hospitals, providers, and state and federal partners. The Rural Health Transformation (RHT) project is supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $195,053,740.44, with 100 percent funded by CMS/HHS.
