The Executive Office of Aging & Independence (AGE) is seeking a mission-driven and dynamic individual to fill the Care Transitions Deputy Director position. The successful candidate must have specific program expertise in care transitions, be experienced in policy development, and program management and be able to communicate clearly conveying and articulating information in writing and orally to AGE managers, senior staff, external stakeholders, and providers. The Care Transitions portfolio includes the Community Transition Liaison Program (CTLP) and Money Follows The Person Demonstration (MFP Demo). These are two of several high visibility initiatives designed to support transition of consumers from institutional to community settings. The Deputy Director will be responsible for day-to-day oversight of CTLP and MFP Demo expanding the programs to meet current policy objectives which are geared to support an individual’s right to choose where they live with support in the community. In this role, the Care Transition Deputy Director works with AGE staff and across Executive Office of Health and Human Services (EOHHS) with other agencies including but not limited to MassHealth Office of Long-Term Services & Supports, the Department of Mental Health, MassAbility, and the Department of Developmental Disabilities to ensure programmatic requirements are met for consumers being served through the Community Transition Liaison Program and Money Follows The Person Demonstration. Reporting to the Director of Home and Community Based Services, the incumbent will support critical aspects of the agency’s mission by ensuring Community Transition Liaison Program and Money Follows The Person Demonstration contractual requirements are adhered to, referrals initiated, services are provided, quality measurements are in compliance, invoices are received and reimbursed and reporting meets expectations as outlined. The Deputy Director oversees annual contracting, budgeting and recommendations for final allowable expenses. The Deputy Director will also take clinical direction from the Clinical Nurse Manager on high visibility nursing facility resident case scenarios.
