DUTIES AND RESPONSIBILITIES: The Day Services Provider is responsible for providing direct care through the implementation of Habilitation Plans. Specific duties and responsibilities include, but are not limited to the subsequent items and are characterized in the following five standards of performance, defined as below: I. JOB KNOWLEDGE Provide supervision and all direct care required to maintain the safety and comfort of the person receiving services Transport or accompany people receiving services on trips to community outings, appointments or recreation activities for the purpose of community integration and exploration Development and planning of inclusive community activities Establish and maintain positive relationships with area businesses Assist people receiving services with completing daily or routine domestic or personal care tasks Foster and promote skills leading to independent living Assist with the development of Habilitation Plans and following these plans for training and assistance Provide training and assist people receiving services to develop, maintain and/or improve desired individual goals, living skills and personal needs as assigned or required Act as role model to people supported Provide supervision to people supported during program hours and during community outings as to ensure their health and safety II. TEAMWORK Encourage and maintain a positive working relationship with all departments in the agency, parents, staff, residents and community partners. Communicate all health, safety, consumer and/or program concerns, sensitive situations and incidents to supervisor and family III. DOCUMENTATION Complete required documentation in conformance with rules, regulations, policies and procedures and billing practices IV. COMPLIANCE & ACCOUNTABILITY Identify and report any untoward incidents Identifying, reporting and/or correcting all safety hazards, including physical plant problems and/or problem work practices and assist with investigating all work-related accidents as requested Participate in any investigations as required Comply with all State and Federal regulations along with agency policies and practices V. TRAINING & DEVELOPMENT Obtain and maintain required training certifications that include CPR and First Aid Attend all required training, staff development programs, and staff/agency meetings as required by the agency and/or State and Federal regulations Attend requested agency-approved training in a related topic that is above and beyond the required agency training
