About The Position

Reporting to the Director of COPD Programs, the COPD Navigator will provide patient education, medical device setup/calibration, and clinical assessments to ensure patients are supported for their respiratory therapy success. How You’ll Make an Impact Become an expert in Hometown Healthcare’s severe COPD product and services portfolio Evaluate respiratory order, understand patients’ prescribed therapy needs, assess and evaluate the patient to begin therapy Ensure proper set-up of medical devices, through clear and thorough patient education, validated by subjective patient responses and objective clinical measures Support total coordination of care lifecycle; patient education, delivery, charting/documentation, care management onboarding, clinical assessments, clinical escalations, and eventual offboarding and asset recovery Participate in 24/7 clinical on-call rotation schedule Expected to be a contributing force in the creation and/or continuous improvement of Standard Work within area of cognizance

Requirements

  • High school diploma or general education degree (GED); or equivalent combination of education and experience - required
  • Certified Respiratory Therapist (CRT) or Registered Respiratory Therapist (RRT) - required
  • Proven ability to communicate clearly, both verbally and written - required
  • Proven success providing an exceptional patient experience - required
  • A history of exceptional attention to detail - required
  • A history of maintaining confidentiality - required
  • Demonstrated mastery responding to questions in a tactful & professional manner - required
  • Strong interpersonal skills & the ability to effectively work with a wide range of individuals - required
  • Demonstrated history of being self-motivated & a desire to work effectively, autonomously - required
  • Proven history of accountability & ownership - required
  • A working knowledge of MS Office Suite and Cloud and/or Google Suite Docs - required

Nice To Haves

  • Associates degree from a 2-year college and/or 4 years related experience or training; or equivalent combination of education and experience - desired
  • A working knowledge of Brightree, and Airview softwares - desired
  • Demonstrated history working in the medical field, or with Home Medical Equipment - desired
  • Success operating within a Quality Management System (QMS) or similar regulated environment - desired
