About The Position

This role is responsible for managing outpatient services to ensure patients receive appropriate care and support. Key responsibilities include scheduling timely post-discharge follow-up appointments, maintaining communication with patients after discharge, and collaborating with healthcare providers to deliver optimal care. Also, maintaining accurate patient records and participating in quality improvement and utilization review initiatives. Coordinators develop, monitor, and assess individual cases, ensuring seamless care coordination with staff. They enhance patient experiences and streamline healthcare processes through effective communication, care coordination, appointment scheduling, and teamwork with healthcare professionals.

Requirements

  • 3- 5 years' Case Management experience in direct Outpatient Care
  • Associates Degree in Nursing
  • Teamwork to coordinate with Doctors, Nurses and other health care personnel
  • Strong verbal communication skills to communicate with patients and the health care team
  • People skills to connect emotionally with patients and health care personnel
  • Leadership

  • Current RN license

Nice To Haves

  • 6 years' Case Management experience in direct Outpatient Care
  • Bachelor of Nursing (BSN) preferred
  • Certification in Case Management

Responsibilities

  • Coordinating outpatient care plans, including scheduling appointments, tests, procedures, and follow-up visits
  • Communicating with patients about their status and condition, including explaining treatments and procedures
  • Working closely with physicians, nurses, and other healthcare providers to ensure the best possible care for patients
  • Maintaining records of patient care, condition, progress and problems in order to report and discuss observations with a healthcare provider
  • Coordinating the referral process, including contacting insurance companies for pre-authorizations
  • Monitoring and evaluating the effectiveness of the care plan, making adjustments as necessary
  • Providing emotional support to patients and their families, helping them understand and manage their condition
  • Ensuring compliance with healthcare regulations and safety standards
  • Organizing and maintaining patient health records for easy retrieval and updating
  • Using data to do analytical tasks related to patient information
  • Maintain productivity levels as defined by the organization, ensuring timely completion of care coordination tasks and adherence to established benchmarks.
