About The Position

Responsible for the documentation of rehab program consumers for all activities and efforts. Supervise staff whose program deliverables are job development, job coaching, DDD services and Long-term Follow Along services. Ensure all funder requirements are met accordingly.  Ensures that program is meeting monthly goals – particularly the grant funded program- Long Term Follow Along (LTFA). Responsible for having up-to-date understanding of the status of Program’s monthly revenue goals and inform Supervisor mid-month of any program’s shortfalls. Develop an action plan to either meet goals by end of the month or ensure that the following month has better results.  Report information to the DVR counselors, job developers, coaches and Supervisor. Generate relationships with referral sources in addition to DVR, CBVI and private funders.

Requirements

  • BA in related field and or HS with at least 4-6 years relevant work experience.
  • Must maintain a valid driver’s license and ongoing access to own transportation.
  • Intermediate Word, Excel and Outlook preferred
  • Excellent verbal and written communication skills
  • Must be able to prioritize and multi-task
  • Navigate and negotiate through difficult situations
  • Ability to communicate with all levels of employees
  • Requires the supervision of 4 direct reports.
  • The Coordinator must work effectively as a Team leader/partner with NJ Services, the job developers, job coaches and other rehab staff.
  • Meet budget goals for job coaching & placement services vouchered services and budget goal for LTFA contract
  • Be able to work from 8:00am to 4:00pm minimum w/flexibility to periodically attend events on weekends or evenings

Responsibilities

  • Supervision and hire of all employment services staff.
  • Provide formal instruction to job developers & coaches regarding job duties.
  • Ensure that staff are: 1) Reading files of consumers who are pending placement or have been placed 2) communicating to staff and DVRS in a timely manner, if needed  3) document and or email  staff efforts in a timely and thorough manner to all pertinent staff/ funders
  • Inform job coaches when placements are made and coordinate the start of job coaching services with the AVP as needed. Ensure staff engagement goals are met monthly.
  • Ensure that consumers who are placed are assigned to a job coach in a timely manner.  Ensure placement goals are met monthly
  • Assists employers with completing appropriate documents in order to benefit from the available DOL and DVRS work incentives.
  • Review all billable/voucher job coaching logs and activity reports for consumers prior to submission for billing.
  • Act as the liaison between staff/employers, DVR counselors, and consumers in order to ensure that all program requirements are being met.
  • Ensure staff are meeting goals and maximizing their time by utilizing Outlook calendars to track their activity.
  • Ensure completion of vouchers and correctly identify billable time for countable services & activities and include request for addition voucher hours if necessary; submit reports to Supervisor to be sent to DVRS in a timely manner; Ensure that reports & coaching requests are completed 
  • Ensure employer and consumer satisfaction with coaching services, surveying stakeholders on a quarterly basis.
  • Document & file documents in participant case files.
  • Visit employment sites in order to ensure that employers are satisfied with job coaching services.
  • Follow up with both consumer and employers after placement into employment in order to ensure smooth transitions to job coaching.
  • Generate weekly and monthly statistical reports internally.
  • Ensure staff attend professional development training 12 hours annually for fulltime staff.
  • Serve as an agency representative at various community and social service meetings.
  • Increase referrals to Goodwill through marketing presentations to outside organizations.
  • Review weekly and monthly status reports relevant to program performance.
  • Receive and review all notes as per the case note and paperwork compliance procedure. Return case notes needing revisions to appropriate staff and monitor the resubmission of the case notes for completion.
  • Submit weekly/monthly reports for departments in a timely manner, as requested by the Director of Rehab.
  • Develop and maintain weekly, monthly, and quarterly reports to be disseminated to program partners and funding source.
  • Complete internal statistical reports and progress reports for funding sources.
  • Create program goals for outcome measurement as required for CARF.
  • Attend CARF sessions/meetings.
  • Input data into Foothold Awards system. Provide oversight of all data in Foothold. Generate reports as needed.
  • Submit quarterly reports on Outcome Measures.
  • Draft final CARF outcome measurement report related to the goals.
  • Meet all budget and placement goals and contract deliverables.
  • Ensure work meets all CARF service and documentation standards as well as Goodwill compliance standards.
  • Provide or assign backup for staff as needed. Fill in for staff as needed.
  • Attend rehabilitation department and all staff meetings.
  • Identify opportunities for program growth and propose new program/service ideas.
  • Coordinator will maintain a caseload. Educate employers about disability issues, ADA issues, tax credits and job modifications.     Ensure that work meets all CARF service and documentation standards as well as Goodwill compliance standards.
  • Ensure adherence to all Rehabilitation and Goodwill policies and procedures.
  • Participate in weekly and monthly meetings with other NNJ staff. Maintain a small caseload of a minimum of 10 cases.
  • Collaborate, provide feedback & participate in case conferences as necessary.
  • Become familiar with labor laws, demand occupations and ADA.
  • Motivate staff to perform to his or her potential.
