About The Position

Position Purpose: Participate as a member of an inter-disciplinary team in the planning and implementing of home and community-based intervention with families and their children, from birth to age five that chose to participate in home visits voluntarily.

Requirements

  • Require a Bachelor’s degree in Early Childhood, Nursing, Social Work, or related field.
  • Required a Minimum of 3 years of experience working with parents and their families of children birth to 5 years of age.
  • Must provide proof of Basic Life Support (BLS Provider-American Heart Association or Professional Rescuer-American Red Cross) prior to employment or within 30 days of hire and maintains current status.
  • Must provide proof of Mandatory Reporting for Child and Dependent Adult Abuse training prior to employment or within 6 months of hire and maintain current status per State of Iowa guidelines
  • Reliable transportation to get to all visits.
  • Understand and respect the cultural differences in families.

Responsibilities

  • Initiates, coordinates, and implements a referral and intake responsibility for Family Connections' new program participants.
  • Assesses, plans, implements and evaluates the needs of families.
  • Imitates, coordinates, and implements the development of the Individual Family Service Plan with program participants and those in attendance.
  • Collaborates with community agencies, community programs, and families.
  • Completes referrals for families as needed.
  • Instructs, models, coaches, and provides ongoing encouragement and support to participating families in identified areas of family need.
