The Admissions Coordinator is responsible for processing new referrals received via telephone, email, fax, and website inquiries, ensuring timely follow-up on pending patient charts daily. This role involves accurately entering information for data integrity and reimbursement, verifying Medicare and Medicaid hospice benefits, and notifying relevant departments about commercial insurances and managed care plans. The coordinator will also inform Marketing Representatives of new referrals outside of standard Program Scheduler hours (8pm-8am local time and weekends). Key duties include contacting patients and families to offer same-day appointments, explaining hospice benefits and services, assigning appointments to VITAS Admissions Representatives, and completing admission consent paperwork. The position requires updating referral sources, maintaining effective relationships with internal and external customers, documenting outcomes from Admissions Representatives, and facilitating Home Medical Equipment delivery. Additionally, the coordinator supports coworkers, mentors new team members, participates in morning check-in meetings, and consistently upholds the VITAS Mission, Core Values, and Care Connection Center cultural platforms.
