Introduction The State Office of Rural Health (SORH) serves as a focal point and state clearinghouse for collection and dissemination of information regarding rural health care issues, research findings, and innovative approaches to the delivery of health care in rural areas; provides coordination of rural health programming across the state to avoid redundancy and support coordination of efforts among public health and health care entities; and delivers technical assistance to rural-serving public and non-profit private entities to build capacity to access state and federal resources. Ultimately, as the state entity primarily responsible for rural health, SORH aims to support the Department, other state agencies, and rural Maryland communities to improve the accessibility of quality health services throughout rural Maryland and positively impact the health of rural Marylanders. The RHTP will improve the health of rural Marylanders by (1) strengthening the rural health workforce, (2) connecting rural Maryland to primary care, chronic disease management, specialty care, and behavioral health, and (3) addressing chronic disease with increased demand for and supply of nutritious food.
