About The Position

Provide case management for individuals with a serious mental illness or substance use disorder in a person-centered, trauma informed manner with a focus on supporting the individual in their recovery process and increase independence.

Requirements

  • Demonstrated skills and competencies in treatment with individuals with a behavioral health diagnosis, demonstrated by at least one of the following: two years lived experience; two years’ direct care experience in a human service field; two years of training in a human service field; or a bachelor’s degree or higher in psychology, sociology, or related human service field, which is preferred.
  • Must be authorized for coverage under agency insurance if operating an agency vehicle. Must have a good driving record, valid Nebraska driver’s license, and/or required level of personal auto insurance.
  • Must be knowledgeable in rehabilitation and recovery principles, coping skills, advocacy techniques, and community resources.
  • Ability to communicate so that dialogue with others can readily occur.
  • Sufficient vision to drive a vehicle and to read and generate documents/correspondence, and complete data entry/documentation.
  • Ability to communicate in spoken conversation, adequate volume speech, and have normal hearing for conversation, telephone use and emergency information systems.
  • Ability to safely maneuver independently on all surfaces of the work area.
  • Have physical stamina to work at least 8 hours per day.
  • Demonstrate ethics that are above reproach by consistently making decisions that prioritize integrity, transparency, and accountability.
  • Well-developed skill in the ability to give recognition or appreciation to others.
  • Ability to plan and coordinate an entire activity/individual service program to include effective teamwork with other service agencies, family members, etc.
  • Demonstrated ability to analyze job and develop functional understanding of technical information and instructions presented in books, manuals and diagrams.
  • Demonstrated ability to perform duties which change frequently.
  • Demonstrate ability to navigate an electronic health record system for documentation.

Responsibilities

  • Maintain a Community Support caseload (1:25) adhering to program documentation best practices.
  • Utilizing a person-centered, strengths-based approach, develop a treatment plan and facilitate the treatment team meeting collaborating with all members to build an effective support system ensuring measurable goals, specific interventions, and a discharge and relapse prevention plan are documented.
  • Provide interventions, in a trauma informed manner, to facilitate active rehabilitation following the treatment plan
  • Provide crisis management in the form of therapeutic support and intervention in time of crisis and develop a crisis relapse prevention plan.
  • Provide monthly face-to-face service coordination and case management to ensure compliance with service definition requirements. Provide active rehabilitation interventions, focusing on daily living activities, education, budgeting, medication adherence, self-administration, relapse prevention, social skills, and community living, ensuring continuity of care.
  • Participate in monthly clinical supervision meetings to review treatment plans, care levels, medical necessity evaluations, and implement recommendations, while also completing chart audit worksheets.
  • Be knowledgeable about diagnostics and medications, including treatment options, reasons for use, and potential complications/side effects.
  • Deliver services using a client-centered approach focused on addressing ambivalence and motivational processes within individuals, facilitating change through identification, examination, and resolution of ambivalence about changing behavior.
  • Utilize Motivational Interviewing techniques to promote participant autonomy and change by fostering relationships with staff, aligning with the participant's values, concerns, and Recovery Vision.
  • Be an active team member by effectively communicating with all team members, in a timely and respectful manner, to ensure quality of program delivery.
  • Adhere to licensure and contract requirements, including CARF, Medicaid regulations, and Division of Behavioral Health’s Service Definitions, ensuring standards are met.
  • Comply with transportation policies and procedures: Follow all state and agency rules and regulations when transporting self and/or a participant. Travel is required for business purposes.
  • Adhere to confidentiality policies and procedures: Process all information in a professional and confidential manner, following HIPPA standards of conduct.
  • Perform work activities in conformance to Goodwill values, immediately report any wrongdoing (unethical/improper conduct, fraud, fiscal mismanagement, etc.), embrace and support diversity, and encourage and support others to do the same.
