This position is embedded in our school-based health center (SBHC). It will support both outreach and care coordination functions, helping students and families access all our services and address social needs. The position works under the direction of the Marketing & Fundraising Manager to promote the utilization and visibility of our services.

Requirements

  • Experience in case management, care coordination, education, outreach, or community health.
  • Excellent communication, organization, and interpersonal skills.
  • Cultural competence and commitment to health equity.
  • Ability to build trust and rapport with diverse students and families.
  • Comfort working in school settings and navigating confidentiality (HIPAA/FERPA).
  • Associate or bachelor's degree in social work, education, communications, public health, or a related field or equivalent work experience.
  • Experience with outreach, public engagement, or marketing in a community setting.
  • Knowledge of health and human services available in Champaign County.
  • Bilingual (Spanish) language required.

Responsibilities

  • Assist students and families in navigating healthcare and school systems.
  • Connecting families to essential services such as behavioral health, medical, psychiatry and dental.
  • Works with case management to refer for assistance with housing, food, and transportation.
  • Support scheduling, consent forms, and follow-up communication.
  • Serve as a liaison among families, SBHC staff, school personnel, and external providers.
  • Ensure coordination of care across multiple systems and services.
  • May be asked to translate as needed.
  • Promote awareness of the School-Based Health Center among students, families, and school staff.
  • Assist with marketing initiatives, school events, and social media content in partnership with the Marketing and Fundraising Manager.
  • Help reduce non-clinical burdens on providers by assisting with referrals, insurance paperwork, and patient follow-ups.
  • Participate in regular training, mentoring, and peer learning as directed by the Marketing Manager.
  • Contribute ideas for improving access and engagement among underserved students and families.
