Works as part of a clinical team to support the treatment goals for youth (ages 4 to 19) and their families who are engaged in intensive, trauma-informed home/community-based services. Advocates for the youth and families to access appropriate and necessary resources from Human Services and similar departments/agencies to improve the quality of their lives. Collaborates with referral sources (DCYF, Court, and FCCP) and other providers (schools, psychiatric providers, and other agencies) involved in the care of the youth.
