About The Position

As a Community Case Manager, you’ll provide person-centered support to children, teens, and adults by meeting them in the community and helping them build skills, manage symptoms, and connect to needed resources. You’ll collaborate closely with families, treatment teams, and community partners. Some evening flexibility may be required, but you set your own schedule.

Requirements

Bachelor’s degree in a human services field or equivalent work experience
Reliable transportation, a clean driving record, and automobile liability insurance
Excellent communication skills, both written and verbal
Basic computer skills
Results of criminal and all other applicable background checks must meet the approval of Pawnee Mental Health Services.

Nice To Haves

Knowledge of the needs and challenges of adults and children who experience severe emotional disturbances
Experience with Microsoft Office

Responsibilities

Provide community-based services in homes, schools, workplaces, shelters, and other natural settings.
Conduct strengths assessments and develop individualized goals with clients and treatment teams.
Offer therapeutic interventions focused on symptom management, coping skills, and daily functioning.
Connect clients and families with needed resources, benefits, and supports across the community.
Maintain accurate, timely documentation and manage your schedule in the electronic health record.
Participate in clinical supervision, crisis coordination when needed, and collaborative team meetings.
