About The Position

Position Purpose At Saint Francis Hospital and Medical Center, the Community Outreach Registered Nurse (RN) supports preventive chronic conditions such as cardiovascular health throughout the communities we serve. This role provides screenings, education, early‑risk identification, and referral support. The RN serves as a frontline resource—promoting wellness, reducing barriers to care, and strengthening connections to primary and preventive services. The position supports the Together for Wellness Clinic as part of a grant‑funded community health initiative. Requirements

  • Current Registered Nurse (RN) license in the state of Connecticut.
  • Minimum of 1 year of clinical nursing experience.
  • The ideal candidate demonstrates strong cardiovascular assessment skills, working knowledge of other chronic conditions, excellent communication abilities, and the ability to provide clear, culturally sensitive education across diverse community environments.

Nice To Haves

  • Experience in community or public health nursing or chronic disease programs.
  • Knowledge of hypertension management and preventive health strategies.
  • BLS certification (with certification support available post‑hire).
  • Experience working with underserved or high‑risk populations.

Responsibilities

  • Conduct blood pressure screenings at the Together for Wellness Clinic and select community outreach sites.
  • Provide hypertension education, including lifestyle guidance, medication adherence, and risk‑reduction strategies.
  • Promote preventive care and encourage follow‑up with primary care providers.
  • Participate in community outreach events to increase awareness of hypertension and available resources.
  • Conduct limited at‑home or virtual visits for Hartford‑area residents.
  • Coordinate referrals to primary or specialty care as needed.
  • Document screenings, education provided, and referrals.
  • Collaborate with community partners and care teams to improve access to hypertension services.
