Jefferson Center is seeking a Community Outreach Liaison to join our recovery-oriented, Mobile MAT team. Our model helps people thrive by continuously revolutionizing healthcare delivery through a data-driven, team-based approach that integrates behavioral health, substance use, and basic needs coordination. Mobile MAT is a recovery team, working in rural areas where clients have challenges around SUD and mental health services. Mobile MAT uses data and firsthand encounters to bring solution-based treatment to this population. Our team members travel to Gilpin, Clear Creek and rural Jefferson County, assessing and providing treatment. As a Community Outreach Liaison, you will play a vital role in connecting individuals to resources, fostering recovery, and addressing barriers to care. This position also supports community partnership to diversify resources/referrals, expose need, provide education and uphold a continuum of care for this population.
