The Community Outreach Coordinator supports Harmony Healthcare Long Island’s mission by coordinating outreach initiatives, events, and partnerships that strengthen relationships with the communities we serve. This role ensures that HHLI’s programs, services, and educational initiatives reach diverse populations—promoting access to care, wellness, and health equity across all sites. Additionally, the coordinator plays a key role in engaging newly assigned patients, supporting registration and scheduling workflows, and providing seamless patient access during community events.
