About The Position

The Community Outreach Coordinator supports Harmony Healthcare Long Island’s mission by coordinating outreach initiatives, events, and partnerships that strengthen relationships with the communities we serve. This role ensures that HHLI’s programs, services, and educational initiatives reach diverse populations—promoting access to care, wellness, and health equity across all sites. Additionally, the coordinator plays a key role in engaging newly assigned patients, supporting registration and scheduling workflows, and providing seamless patient access during community events.  The following statements reflect the general duties, responsibilities and competencies considered necessary to perform the essential functions of the job and should not be considered as a detailed description of all the work requirements of this position.

Requirements

  • Associate’s or bachelor’s degree in public health, Communications, Marketing, or related field (or equivalent experience).
  • 1–3 years of experience in community outreach, public health, patient engagement, marketing support, or event coordination preferred.
  • Strong communication, organization, and interpersonal skills.
  • Ability to design basic marketing materials (flyers, handouts, ID cards); experience with Canva or similar platforms preferred.
  • Ability to work independently and as part of a team.
  • Flexible schedule with availability for evenings and weekends as needed.

Nice To Haves

  • Bilingual (English/Spanish or English/Creole) strongly preferred.

Responsibilities

  • Plan, coordinate, and participate in community events, health fairs, and educational workshops.
  • Represent HHLI at public events, schools, and community partner meetings.
  • Develop and maintain a calendar of community outreach activities, including scheduling, logistics, and materials preparation.
  • Ensure HHLI’s visibility through event setup, tabling, signage, and branded materials.
  • Track attendance, engagement, and outcomes for all outreach initiatives.
  • Build and sustain strong relationships with local organizations, schools, faith-based institutions, and community groups.
  • Work collaboratively with the Community and Advocacy Liaison to align outreach efforts with advocacy priorities.
  • Coordinate with internal departments (Clinical, Quality, Marketing, Patient Experience, etc.) to support outreach events and initiatives.
  • Identify opportunities for partnership and joint programming to expand HHLI’s community impact.
  • Assist in distributing health education materials and promoting preventive health programs.
  • Support initiatives that raise awareness about HHLI services, health screenings, and special campaigns (e.g., back-to-school events, vaccination drives, wellness fairs).
  • Facilitate patient communication by gathering feedback, assessing needs, and strengthening their connection to HHLI.
  • Conduct direct outreach to patients and/or caregivers through phone calls, in-person conversations, or community-based strategies to ensure continuity of care.
  • Engage community members in conversations about health needs and provide information on HHLI programs and services.
  • Facilitate conversations and listening sessions to elevate patient and community voices.
  • Conduct outreach to newly assigned patients, introducing HHLI services and encouraging them to establish care.
  • Register new patients in eClinicalWorks (eCW) and assist with updating demographic and insurance information.
  • Schedule medical, dental, behavioral health, and other appointments as appropriate.
  • Provide on-site registration and appointment scheduling during community events, ensuring a seamless and welcoming experience.
  • Coordinate follow-up outreach to support appointment reminders, care continuity, and engagement with HHLI services.
  • Design and create marketing flyers, outreach materials, and digital content that promote HHLI services, programs, and events.
  • Develop, update, and maintain HHLI patient identification cards, ensuring accuracy, consistency, and alignment with branding standards.
  • Collaborate with the Marketing team to ensure materials meet organizational branding and communication guidelines.
  • Support the creation of signage, handouts, presentations, and other visual materials used for outreach, education, and engagement.
  • Maintain accurate records of events, partnerships, outreach activities, and patient engagement metrics.
  • Support the preparation of monthly and quarterly reports summarizing community engagement outcomes and impact.
  • Gather community feedback to evaluate program effectiveness and identify new outreach opportunities.
