Dignity Health Medical Foundation, established in 1993, is a California nonprofit public benefit corporation with care centers throughout California, and an affiliate of Dignity Health, one of the largest health systems in the nation. The Community Outreach Coordinator represents Dignity Health and collaborates with the Community Outreach Manager to develop and implement initiatives that increase program visibility, educate the public, and build strategic, collaborative relationships with other community-based organizations to reach more youth in local neighborhoods. The role requires a self-motivated, enthusiastic individual aware of local community resources and experienced in cultivating relationships with community-based organizations.

Requirements

  • Bachelor's degree
  • 2+ years of behavioral health and non profit experience
  • Ability to manage multiple projects simultaneously.
  • Excellent written communication and verbal presentation.
  • Excellent attention to detail and organizational skills.

  • Ability to prepare and present reports on development and outreach activities, and to track and assess progress and address program deficiencies as they occur.
  • Bilingual preferred (Spanish)

Responsibilities

  • Establish relationships with schools and other community partners to support collaboration between Dignity Health and other service providers in targeted neighborhoods.
  • Assist in planning and completing all approved events and activities.
  • Identify and work to eliminate or work around systemic barriers to youth and family participation in agency services.
  • Collect, organize, maintain, disseminate, and make accessible community resource and referral information.
  • Meet regularly with peers, clinicians, supervisors, and Program Manager as needed to consult on client care and access issues that may be addressed through the outreach program.
  • Serve as a liaison between the Clinics and families/youth, providing information about the program, information about the County Mental Health Plan, information on Medi-Cal eligibility and enrollment, and when appropriate, assist in Medi-Cal eligibility verification and enrollment.
  • Represent Dignity Health at events including but not limited to community fairs, educational forums, and collaborative meetings.
  • Schedule outreach presentations in the community and communicate with staff regarding coverage and responsibilities.
  • Track and report outreach outcomes (i.e., number in attendance, effectiveness).
  • Assist in the coordination of special events for both youth/families and staff appreciation activities.
