About The Position

The Community Liaison creates and builds relationships within the Hospice Alliance service area with the purpose of generating referrals for hospice and palliative services. The Community Liaison will be responsible for communicating Hospice Alliance services to prospective patients and their families. The Community Liaison must be articulate, knowledgeable of the Hospice Medicare benefit as well as Palliative care and understand the importance of positively impacting patients and families at this critical time in the patient’s care. Additionally, the Community Liaison will cultivate relationships with healthcare providers including by not limited to doctor offices, hospitals, assisted living facilities, skilled nursing facilities and other service providers such as home care agencies. Excellent relationship building skills and a dynamic/positive attitude are necessary to be successful in this position.

Requirements

  • Bachelor’s Degree in business, communications, healthcare administration or a related field preferred.
  • Ability to work independently with minimal supervision.
  • Ability to resolve problems and set priorities.
  • Excellent written and verbal skills to effectively communicate needs to the public and to field and office staff.
  • Ability to work cooperatively with others, outgoing and personable.
  • Experience in healthcare industry, hospice experience preferred.
  • Proficiency in a database application, Microsoft Word, Excel and a working knowledge of PowerPoint.
  • Successful completion of a pre-employment drug screen.
  • Reliable transportation, valid driver's license and auto liability insurance is required for travel within the Hospice Alliance service area.

Responsibilities

  • In accordance with the marketing plan, market Hospice Alliance services by visiting assigned accounts and meeting face-to-face with physicians, healthcare facilities or other key staff members who influence the choice of hospice provider.
  • Develop and cultivate new prospects, with the goal of adding new people & facilities to increase referral sources and grow census.
  • Maintain a database of accounts and weekly report of referral contact activity. Identify referral patterns and use the data to create opportunities to partner with (or add value to) referral sources.
  • Plan and implement projects or programs that increase the marketing effectiveness of Hospice Alliance.
  • Plan and implement community outreach events/talks to increase brand awareness of Hospice Alliance.
  • Actively participate in any assigned internal and/or external committees/groups.
