About The Position

DHHS is seeking an experience public health professional to support effective implementation of a statewide community health worker initiative across diverse partners to strengthen coordination, consistency, and accountability within a hub-and-spoke model and provide practical technical assistance to partners while centering equity and community voice development. The Program Manager will support the implementation, coordination, and continuous improvement of the statewide Community Health Worker (CHW) initiative.

Requirements

  • Bachelor’s degree in business or public administration, social/behavioral sciences, education, health sciences, public health, law or related field and experience in program planning, implementation, monitoring, evaluating program functions or supervising/management staff and budgets.
  • Related coursework, training and/or education may substitute for the Bachelor’s degree on a year-for year basis.
  • Knowledge Public health systems and community-based service delivery models
  • Public Health principles and social determinants of health
  • Grant-funded program administration and contract monitoring
  • Community Health Worker models (or willingness to learn)
  • Skills Strong project and program management skills
  • Ability to manage multiple partners, timelines, and priorities
  • Clear written and verbal communication
  • Facilitation and relationship-building across diverse stakeholders
  • Ability to translate policy or guidance into practical implementation steps
  • Data-informed decision making and basic performance monitoring
  • Abilities Work independently while collaborating within a team environment

Nice To Haves

  • Experience working with community-based organizations, local health departments, or Tribal programs
  • Familiarity with CHW programs, workforce development, or community-based health initiatives
  • Experience supporting implementation of multi-site or regional initiatives
  • Background in public health, social services, community development, or a related field
  • Lived experience in or strong connection to communities disproportionately impacted by health inequities
  • Experience working with Tribal Nations or understanding of Tribal sovereignty

Responsibilities

  • Coordinate implementation of CHW activities across multiple hubs (local health departments, Tribal Nations, and community-based organizations)
  • Serve as a primary liaison between the state office and hub partners to support contract compliance, workplan execution, and reporting
  • Provide technical assistance to hubs related to CHW roles, scope of practice, supervision structures, and implementation expectations
  • Monitor program performance, timelines, and deliverables; support corrective actions as needed
  • Support data collection, documentation, and reporting requirements in collaboration with evaluation and performance management partners
  • Assist with development and refinement of guidance documents, playbooks, and implementation tools
  • Facilitate meetings, learning communities, and technical assistance sessions with hub partners
  • Contribute to continuous learning, sustainability planning, and scaling strategies for the CHW workforce
