About The Position

The Medical Outreach Assistant serves as a liaison between the patient/family and the health systems providing navigation to facilitate access to care, resources and improved communication to assure patient follow-up, long-term chronic care management and reduce health needs.

Requirements

• Prior experience working in a healthcare and/or community health setting or on-the-job training and orientation in health sciences, social services or related field.
• High School Diploma or equivalent
• May work all three shifts, weekends and holidays.
• Individual shift rotation and schedules will be established with the Manager who will determine the needs for appropriate coverage of services.
• Schedules, including requirements of shift rotations and hours of work, including weekends and holidays, may be adjusted as necessary to meet program and work requirements and/or as needed.
• Physical - Lifting 20-50 lbs.
• Activities: Prolonged standing, Prolonged sitting, Walking (distance), Repetitive motion
• Mental/Sensory: Strong recall, Reasoning, Problem solving, Hearing, Speak clearly, Write legibly, Reading, Logical thinking
• Emotional: Fast pace environment, Steady pace, Able to handle multiple priorities, Frequent and intense customer interactions, Noisy environment, Able to adapt to frequent change

Nice To Haves

• Two (2) years experience working in community service, health or social service sector
• Experience working with underserved populations
• Associate or Bachelors Degree in Health Sciences or Social Services from an accredited program

Responsibilities

• Sets goals and plans to accomplish role/responsibilities.
• Able to handle multiple priorities and manage time to meet workload goals.
• Documents interactions with clients in appropriate format and software.
• Outreach services may be conducted by face-to-face contacts during home or community visits, telephone contact and/or written communication.
