About The Position

Reporting to the Program Manager for Cancer Care, Community Health Worker for Cancer Care Prevention Program is the community ambassador, educator, and navigator for cancer prevention at Fair Haven Community Health Care (FHCHC). The role coordinates FHCHC’s patient-facing efforts at preventing cancers of all types, including breast, cervical, colorectal, and lung. The role effectively communicates about cancer prevention in ways that resonate with communities served by FHCHC. This is a grant funded position with expectation for continued support.

Requirements

  • High School Diploma or GED is required, Associate’s Degree preferred. Experience in community outreach, healthcare, or social services is required.
  • Bilingual English/Spanish is required.
  • Proficiency with Microsoft office, including Outlook, and Excel is also required.
  • Experience using electronic health records, specifically Epic, is highly desirable.

Responsibilities

  • Conduct outreach and education to identified populations in both clinical and community settings
  • Support patients in accessing age and risk appropriate cancer screening services across multiple cancer types
  • Provide navigation services from initial patient identification through completion of screening, and connection to treatment if necessary
  • Assess patients’ readiness to change health behaviors and provide appropriate coaching and support
  • Identify and address any barriers to care, including transportation, food or financial concerns using a standardized screening tool
  • Follow up with patients via phone, in-person visits, and other methods as needed
  • Collaborate with clinical teams to ensure coordinated care
  • Participate in community engagement activities with local partners and organizations
  • Work with marketing department to develop innovative strategies for education, outreach and engagement
  • Document all patient interactions accurately in the electronic health record (Epic)
  • Ensure compliance with HIPAA and protect patient confidentiality
  • Attend team meetings, trainings, and other organizational activities as required
  • Performs other duties as directed by supervisor
