About The Position

The Community Health Care Worker will work to screen all patients for Social Drivers of Health, including but not limited to food insecurity, housing, transportation, utility insecurity. The Community Health Care Worker will build up an external community based resource network and connect patients to the appropriate resources. The Community Health Care Worker will document results and make necessary internal referrals.

Requirements

High School Diploma or equivalent
Community Health Worker Certificate - or to be obtained within the first 6 months of employment
Quick learner and self-starter
Works well independently and within a team
Familiarity with data entry and tracking, basic computer skills
Excellent verbal and written skills; presentation skills a plus
Experience working and/or engaging in priority neighborhoods
Strong interpersonal skills and the ability to build trust and maintain relationships with community partners, as well as patients.
Strong organizational skills with good follow-through to complete assignments within specified timeframes
Demonstrate ability to work collaboratively
Demonstrate ability to work with families who face exceptional challenges
One year of experience in community work preferred.

Responsibilities

Conduct community-based assessments and interview to identify and prioritize patient social services needs and objectives
Responsible for screening patients for social drivers of health including but not limited to food insecurity, housing, transportation and utility insecurity.
Build relationships with community based organizations that offer resources and services to address social drivers of health.
Connect patients identified with social driver of health needs with community based organizations.
Follow up with patients to make sure they were connected with the needed resources.
Work with the care coordination team to provide additional resources to patients as needed.
Track all assessments and outcomes in the designated electronic medical record.
Establish and maintain positive relationships with patients, visitors, and other employees
Contributes to and consistently applies Catholic Health policies, procedures, and benefits to all customers and/or employees without discrimination.
