About The Position

The Connected Care Community Health Worker provides information, resources, and care coordination to help older adult patients and their families access the care they want and need. They work closely with patients primary care and behavioral health providers, communicating key information about what matters most to patients. Responsibilities include assisting patients with completing Advance Directives, reconciling medications, identifying and removing fall hazards in the home, providing self-management support and education for chronic conditions, and connecting patients with community resources. The role also includes traveling to patients homes to provide care and support. This is a pilot program, grant-funded through 2026 and a full-time position based in White Salmon, WA, with the possibility of working in Stevenson, WA, depending on patient needs. Paid on-the-job training is provided for candidates not yet certified as a Community Health Worker (CHW). Certification can take up to six months, after which employees will receive CHW certification and an increase in hourly pay.

Requirements

  • High School Diploma or GED.
  • 2 years experience in a healthcare or social service-related field.
  • Valid driver's license and clean driving record.
  • Willingness to travel for events, paitent care, and between clinic locations, as needed
  • Experience giving presentations and provide instruction to patients and groups.
  • Familiar with medical terminology and the healthcare system including the Oregon Health Plan and Medicare.
  • Knowledge of community resources in OCH services areas, especially those available to older adults.
  • High-level computer skills with Microsoft Office applications, Word, Excel, Outlook, Epic, and other systems.
  • Knowledge of and compliance with HIPAA regulations.
  • Work independently, work with multiple projects and maintain a high degree of professionalism and confidentiality.

Nice To Haves

  • Fluent Bilingual in English/Spanish.
  • Advanced education such as graduate of an accredited Medical Assisting program.
  • 90-hour Community Health Worker certification or Certified Community Health Worker
