About The Position

White Oak Medical Center seeks to hire an experienced Community Health Worker for our Care Navigation Department who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing. As a Community Health Worker you will: In collaboration with the Transitional Care RN, coordinate patient care with patient, family, and ancillary/community services to promote quality and cost-efficient care for assigned caseload. Assist patients with enrollment and/or referring them to various health and social service programs. May include meeting patient in community to assist with connection when needed. Assist patients to schedule medical appointments to include connection with transportation resources. Provide follow-up to patients, by phone, home visits and visits to other settings where patients can be found. Conduct home visit to ensure that the patient has what is required to recover safely at home. Visit to include safety checks (e.g., check for fall risks) and preparation for follow-up appointments with provider.

Requirements

  • Bachelor of Science in health education, health promotion, or related field
  • 1 - 2 years of experience working in a community-based setting with knowledge of local resources and system navigation
  • Licensed driver and able to operate a motor vehicle as well as maintain automobile insurance
  • Active American Heart Association Basic Life Support (BLS) certification required
