The Community Health Worker positions are part of the CHW Academy which is a 13-month program where individuals are in a classroom setting for initial learning of what it means to be a Community Health Worker and then transitions to worksites across Jefferson Health and in the community. At completion of the program, person meets requirements for Community Health Worker certification in PA. Job Description The Community Health Worker (CHW) serves as a bridge between healthcare providers, social services, and the community, with a focus on improving health outcomes for individuals, families, and communities. CHWs provide essential support through outreach, health education, resource coordination, and advocacy, particularly for underserved populations. CHW will provide social support, complete Health Related Social Needs (HRSN) screening, informal counseling, and advocate for individual and community health needs. Home and community-based visits are work assignment specific but could be an essential function of a CHW’s work. This role requires cultural competency and a commitment to empowering individuals to improve their health and well-being. Location assignment and work settings could include: outpatient care areas, patient homes, inpatient settings, skilled nursing, community organizations, and participating in community events. Participation in community events and some evening/weekend hours may be required.
