About The Position

Meritus Health Plan is seeking a motivated and community‑focused Community Health Program Coordinator to support local health initiatives and strengthen partnerships across our service area. In this role, you will help assess community needs, support collaborative projects, and deliver meaningful health education and outreach programs.

Requirements

  • Associate degree or higher required.
  • At least 1 year of experience in a community‑based or nonprofit organization in an administrative or clerical role.
  • Ability to plan, implement, and evaluate community health promotion events.
  • Strong administrative, interpersonal, and communication skills.
  • Ability to work independently and as part of a team.
  • Ability to travel as needed.
  • Intermediate proficiency in Microsoft Word, Excel, and PowerPoint.

Nice To Haves

  • Experience in a managed care environment preferred.
  • Knowledge of community‑based programs within the Meritus Health Plan service area preferred.

Responsibilities

  • Support and evaluate community‑based projects and identify opportunities for new partnerships.
  • Serve as a communication link between community organizations and internal departments.
  • Assist in developing and implementing community‑focused programs and health awareness initiatives.
  • Work with subject matter experts to create and deliver health education materials.
  • Organize, support, and participate in community health promotion events.
  • Represent Meritus Health Plan on community boards or committees to advance organizational goals.
