About The Position

The Community Engagement Specialist (CES) Team Leader II oversees management and implementation of a grant awarded to coordinate care of consumers with serious mental illness, substance abuse, co-occurring or co-existing disorders that are at risk of psychiatric hospitalization or are currently hospitalized. The role involves promoting service excellence, mentoring staff, collaborating with leadership, and ensuring effective service delivery.

Requirements

  • Minimum of BA required
  • Minimum 1 year related experience
  • Must have a high level of interpersonal skills to handle sensitive and confidential situations.
  • Possess strong written and verbal communication skills in conjunction with strong time management and organizational skills with the ability to complete multiple tasks and high volume of work with deadlines.
  • Must be able to lift 25 lbs.
  • Valid Driver’s license required.
  • Must complete CPR and CPI courses per agency policy.

Nice To Haves

  • 1 year of supervisory experience preferred

Responsibilities

  • Promotes the principles of Service Excellence in all actions and activities.
  • Provides mentoring as needed to ensure quality of care and consistency in practice.
  • Collaborates with department leadership in the care delivered to consumers to ensure seamless, effective service delivery.
  • Provides monthly supervision with assigned staff to assure that all processes are in place and being followed.
  • Supervises assigned staff, evaluates staff performance, establishes, and maintains employee work schedules and timekeeping activities, and makes recommendations for personnel actions as needed.
  • Trains and mentors CES staff.
  • Ensures all requirements of CES Statement of Work (SOW) are met.
  • Ensures face to face contact with a consumer within 3 days of discharge from a psychiatric hospitalization.
  • Ensures crisis safety plans are completed with consumers.
  • Refers consumers and families to additional services based on individual consumer need.
  • Assists/links consumers to personal and community supports necessary to live independently in the community such as housing, employment, benefits, paying bills, etc.
  • Provides outreach in the community served to promote community engagement services.
  • Provides after hours availability to meet individual consumer needs.
  • Engages and communicates effectively with consumers and families, physicians, health care providers, internal and external team members.
  • Responds to all forms of communication (IM, telephone, email) within a timely fashion.
  • Orders supplies as needed.
  • Demonstrates service excellence skills with internal and external customers.
  • Attends all departmental and center meetings/trainings as assigned.
  • Adheres to confidentiality, HIPAA, and risk management policies and procedures including but not limited to completing Incident Reports.
  • Participates in other functional work by participating in related projects, as applicable and directed by Center leadership.
