About The Position

Provides assistance and supervision to individuals in the least restrictive treatment model in all programmatic concerns. Assists in delivering services to persons with disabilities as required and recommended in the Person-Centered Plan (PCP). Provides personal care as needed: assistance with toileting, dining, bathing, etc. Works cooperatively as a team member, to provide the best possible services to individuals. Completes documentation of responses/daily activities to training Provides assistance in directing consumers to make responsible choices regarding their individual rights in all aspects of their program. Plans and facilitates individual and/or group activities both on site and in the community. May be assigned to work with individuals across the LEC both on site and in the community depending on the needs of the individual(s). Position may be responsible for subbing as a driving and/or being an aide on a van/bus, transporting consumers to/from Meaningful Day Program/ the LEC. (See appropriate aide or driver job description for additional duties.) If needed, transports individuals to all off-site activities including to outings. Assists consumers to make choices about goals/outcomes, services, etc. that they would like included in their Person-Centered Plan by the designated deadline prior to their PCP Meeting. Attends PCP meetings to review the consumer’s progress and provides a current level of support needed in a professional manner. Ensures Meaningful Day services are provided to consumers as required and recommended in their Person-Centered Plan. Develops small group activities (facility based and community based) which cover a range of issues related to employment, education, recreation, safety, leisure, etc. Documents progress on all aspects of the PCP. Serves as an adult/peer role model to employees for normative interpersonal relations and fosters an atmosphere of cooperation and respect. Utilizes appropriate lines of authority to address personnel issues, concerns, questions, etc. Performs all aspects of medication administration and documentation, as directed. Provides appropriate training and guidance, as needed, to the Community Development Assistants working with him/her. Displays sound judgment in exercising responsibility for the safety and well being of the consumers and maintain a safe work environment at all times. Reports all health and/or safety related incidents in accordance with P&P and state regulations. Ensures that consumers have access to appropriate protective and cleaning supplies by assessing facility based and community-based material needs. Maintains accurate, up-to-date records of employee’s production information including time studies, as needed and required. Is expected to demonstrate knowledge of their job in a conscientious manner with minimal supervision under routine circumstances. Advocates for consumer’s human rights in all program decisions. Maintains confidentiality in all matters related to the consumers’ program and personal circumstances. Assists program with Quality Assurance activities which includes assistance with annual Satisfaction Surveys. Actively participates in staff meetings and attends all mandatory agency and DDA staff training. Participates in marketing and public relations efforts, as requested. Participates in employer networking opportunities, as requested. Performs other duties as assigned by Supervisor.

Requirements

  • HS diploma or G.E.D.
  • Ability to read and interpret regulations.
  • Good verbal, written, and interpersonal skills.
  • Good time management and organizational skills.
  • Ability to receive and maintain certification as a Certified Medication Technician (CMT
  • Ability to receive DOT certification, when required.
  • Ability to learn and effectively use software applications within agency-designated timeframe.
  • Must possess typing skills sufficient to be productive.
  • Valid drivers license with a minimum of 3 years of driving experience and an agency approved driving record.
  • Must be at least 19 years of age.
