About The Position

The Community Connections Case Manager maintains and strengthens family units, by providing intensive in-home services designed to prevent children from entering the foster care system. This program, works with family strengths, monitors the family dynamics, provides linkages to community resources, assists with immediate needs and concerns, as well as general case management services. Emphasis is on child safety and keeping the family together.

Requirements

  • Bachelor's’s Degree in Social Work or related human services field (Related fields will be considered.)
  • 0-6 months case management experience preferred but not required
  • Child Welfare Employee License and any additional certifications as mandated by the Department of Children and Family Services.
  • Knowledge of administrative and office procedures and systems such as word processing, managing files and records, stenography and transcription, designing forms, and workplace terminology.
  • Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
  • Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.
  • Knowledge of group behavior and dynamics, societal trends and influences, human migrations, ethnicity, cultures, and their history and origins.
  • Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions, and for career counseling and guidance.

  • Recognizes how trauma affects everyone involved-including clients, their families, staff, and community; and impacts behaviors.
  • Responds to trauma by actively implementing the guiding principles of trauma informed care into their daily practice.
  • Resists re-traumatization of persons served and staff providing services, by creating safe, welcoming, and supportive environments.
  • Proficient Computer Skills In: Outlook, Word, Database Management Software
  • Access to reliable transportation to get to/from work
  • Access to a reliable automobile
  • Must provide proof of auto liability insurance (and continue to while working in this position)
  • Must possess a valid driver's license

Responsibilities

  • Meets with families in their homes at various hours of the day as well as on call duties as scheduled in potentially high-risk neighborhoods.
  • Facilitate connections to community-based resources and supportive services to address challenges impacting family stability and well-being.
  • Serves as a conduit of information between all parties to the case and identified stakeholders, including medical, educational and mental health providers.
  • Conducts visits with children, parents and/or caregivers that include the on-going assessment of child safety, permanency and well-being.
  • Develops & implements a plan for delivery of services & supports in collaboration with the family that meets the immediate needs of the children and the family
  • Performs case management responsibilities in accordance with state and federal laws, Department of Children and Families Administrative Codes and Operating Procedures, funding source policies and internal policies on child abuse, abandonment, and neglect within required timeframes.
  • Willingness to travel as position requires frequent travel; follows agency and state guidelines regarding driving, rules of the road, and safety.
