About The Position

SHARE (Supportive Housing Assistance to Reach Excellence) is a supporting housing service designed to assist persons with serious mental illness(es) in maintaining independent housing in the community. The case managers provide the support needed in each person's home environment and in the community. SHARE provides outreach and case management services to program participants. The clinician works with these individuals in the community to complete assessments and develop an Individualized Rehabilitation Plan (IRP). Additionally, the clinician provides clinical oversight for the case managers to support consumers.

Requirements

Graduation from an accredited college or university in Social Work, rehabilitation counseling, psychology, criminal justice, counseling, or other related behavioral healthcare or counseling
LAC/LSW required
One year of experience in the psychiatric/mental health field
Graduated from an accredited college or university in Social Work, rehabilitation counseling, psychology, criminal justice, counseling, or other related behavioral healthcare or counseling
Valid N.J. drivers license with less than 6 points

LPC/LCSW preferred

Responsibilities

Perform therapeutic rehabilitative skill development
Maintain accurate documentation and client records
Recommend referral services as needed
Develop, implement and maintain client IRP
Provide education to client systems related to nutrition, symptom management, medication safety and management, and discharge planning
Provide transportation to clients as needed
Provide crisis intervention services as needed
