About The Position

Frontier Behavioral Health, a non-profit organization with over 100 years of presence in Spokane, is dedicated to providing evidence-based treatment to adults, youth, and seniors facing a wide range of behavioral health issues. The Forensic PATH team specifically works with individuals involved in the forensic mental health system, assisting them in identifying, locating, and accessing community resources within Region 1 of Washington State (Spokane, Stevens, Ferry, Pend Oreille, Lincoln, and Adams counties). The Clinician I is part of a multi-disciplinary team, including peers, a CDP, and clinical staff at Bachelor's and Master's levels. This team delivers outreach and engagement services to individuals with criminal justice system involvement or those identified as high utilizers of hospital emergency services, who are on the Health Care Authority (HCA) referral list. The FPATH Team collaborates closely with Outpatient Competency Restoration, Forensic Navigators, and Forensic HARPS programs to engage clients and provide case management, leveraging existing health and behavioral health benefits to enhance services.

Requirements

  • Bachelor's degree in psychology, counseling, social work or related field from accredited college or university
  • Must apply for a Counselor Agency Affiliated – Registered credential from the WA State Department of Health within 30 days of hire
  • Must have an unrestricted and valid driver’s license, be able to pass a motor vehicle screening and driving test, have active car insurance, and be willing to use their personal vehicle in the course of work
  • Ability to communicate and engage effectively with individuals of varied, backgrounds, trauma experiences, cultures, education level and socio-economical status

Nice To Haves

  • Two years of experience working with psychiatrically impaired populations
  • Experience in the criminal justice system

Responsibilities

  • Work with individuals who have been identified by the Health Care Authority as eligible for the program
  • Assess needs of individuals and provide case management and assistance in connection to resources
  • Assist with access to employment and educational services
  • Assist with Social Security benefits application (SOAR-trained staff)
  • Utilize Client Support Funds for basic needs or to assist with engagement to resources
  • Locate individuals identified on the HCA referral list
  • Create or further develop recovery plans and on-going support to promote progress in care
  • Provision crisis and hospital diversion services to individuals at high risk
  • Complete suicide and homicide risk assessments to determine a level of response needed to maintain the safety of the client or others
  • Assist non-Medicaid individuals to access Medicaid as appropriate
  • Assist with obtaining SOAR services
  • Provide evaluation and treatment for patients who have behavioral health concerns
  • Assist and complete care plans and provide assessments and referrals for outpatient or inpatient treatment
  • Provide consultation, training, and education to staff regarding behavioral health issues as assigned by supervisor
  • Facilitate use of/access to crisis hospital diversion resources, including Stabilization and/or E&T admission
  • Identify and include the client’s support network in care coordination
  • Assist clients in identifying and accessing resources which will meet their needs
  • Coordinate and collaborate with community resources on behalf of clients
  • Maintain client records and all other required documentation in a timely and confidential manner, in accordance with agency policies and practices
  • Requires travel to see clients in the community and travel between work sites either via FBH owned Vehicles and/or personal vehicle
  • Other duties as assigned
