About The Position

Serving more than 10,000 Georgians, River Edge continues to be one of the state's leading resources for mental health, substance use disorder treatments and supports as well as individuals with intellectual and development disabilities. River Edge offers outpatient services at our Emery Highway-Macon, Monroe County and Baldwin County locations. Outpatient services include behavioral health, nursing, and psychiatric assessment, stabilization and medication maintenance services as well as crisis intervention services as well as awareness, education and recovery groups. Clinician – Adult MH Residential Program Overview: Community Residential Rehabilitation (CRR) is a highly intensive, outreached based, out-of-climbing service provided by a multidisciplinary team of professionals. Population targeted is individuals who have serious and persistent mental illness, and most have a co-occurring diagnosis of substance abuse. These are individuals are seen in the residential setting and in the clinic setting for treatment. Currently, the CRR settings are in Baldwin County under the CRR 1 and CRR 3 service line categories. Position Overview: River Edge is currently seeking a Licensed or License-Eligible Professional in Social Work, Marriage and Family Therapy, or Professional Counseling to serve as a Clinician for our Adult MH Residential clients. In this role, you will play a vital part in supporting the Lead Clinician and collaborating with Case Management, the ACT Team, and other service providers to ensure the delivery of high-quality, person-centered mental health services. Requirements

  • Licensed professional (LAPC, LMSW, LAMFT, LMFT, LPC, LCSW) or license-eligible professional in the social service field.
  • Master’s degree in social service field (Social Work, Sociology, Psychology, Human Services) or a related discipline.
  • Minimum of one (1) year of post-master’s experience providing mental health and substance use treatment (preferred).

Responsibilities

  • Conduct and facilitate in individual assessment, treatment planning, counseling, referrals, and transition/discharge planning.
  • Conduct confidential Comprehensive Needs Assessment (Biopsychosocial Assessment) to determine the appropriate level of care for clients.
  • Develop a client/person- centered Individualized Recovery Plan (Plan of Care) that aligns with identified services and referrals.
  • Collaborate with clients in development of a Housing Goals along with completing the Housing Choice & Needs Evaluation.
  • Collaborate with clients and assign providers in development of Primary and Secondary Transition/Discharge Plans.
  • In partnership with clients, develop Residential Crisis Response Plans.
  • Complete collaborative documentation:
  • Identifies and provides intervention for potential crisis situations.
  • Attend staff meetings, case conferences, and training programs as designed.
  • Attend clinical supervision to increase skills and knowledge.
  • Maintain working knowledge of treatment agencies and programs and a list of resources that can be shared with clients.
