JOB SUMMARY: The Clinical Specialist is a mental health professional who reports directly to the Division VP. This position is primarily clinical and consultative in nature and provides services to all supported housing programs (both single- and scatter site) throughout the cluster. While the Clinical Specialist does not carry formal supervisory or operational responsibilities, the VP may deputize him/her to delegate and monitor certain tasks and responsibilities to staff at the Program Director level and below in relevant Supported Housing (SH) programs. ESSENTIAL JOB FUNCTIONS: To perform this role successfully, an individual must be able to perform each essential duty satisfactorily, with or without reasonable accommodation: Deliver clinical consultation and crisis management support to supported housing teams. Consult on challenging cases (e.g., clients with co-morbidities, psychiatric risk, AOT/forensic involvement), and maintain a "clinical watch list" of such cases. Lead internal case conferences to support staff in designing person-centered, evidence-based interventions. Represent the program at external case conferences related to discharge and transition planning. Conduct and monitor risk assessments, ensuring follow-up and communication with Program Directors and leadership. Provide or coordinate staff training in clinical topics, independent living skills, wellness, etc. Review clinical documentation (e.g., support plans, progress notes, risk assessments) for quality and compliance. Participate in Clinical High-Risk and Supported Housing team meetings. Support assessments of consumer readiness for supported housing and guide training needs. Participate in discharge planning and case conferences. Conduct housing interviews for prospective tenants. Coordinate with Health Connect for tracking and engagement of high-risk clients. Perform other job-related duties as assigned. Ability to work collaboratively within a multidisciplinary team. Effective multitasking, project coordination, and task delegation. Problem-solving and critical-thinking skills. Cultural competency and respectful communication with clients and staff. Strong clinical assessment and intervention planning skills.
