About The Position

The Hospice Clinical Specialist partners closely with regional and agency hospice leadership to strengthen clinical practice, regulatory compliance, survey readiness, quality outcomes, and operational consistency across all hospice agencies. This role supports implementation of standardized clinical care models, provides expert guidance on Conditions of Participation, and contributes directly to quality, regulatory, and performance improvement initiatives. The Specialist serves as a subject matter expert on hospice clinical operations and is a key contributor to organizational growth, sustainability, and excellence in patient and family care.

Requirements

  • Minimum two years of hospice clinical experience and two years of hospice operational, quality, or compliance support experience.
  • Deep knowledge of Medicare, Medicaid, state regulations, and hospice regulatory compliance.
  • Experience with QAPI, performance improvement, and clinical quality monitoring systems.
  • Strong analytical, problem-solving, and documentation review skills.
  • Comfortable learning and optimizing technology systems, especially Homecare Homebase.
  • Strong communication, organization, and interpersonal skills.
  • Demonstrated ability to work across multiple sites and manage competing priorities.
  • Valid driver’s license and auto insurance (must be listed on policy).
  • Ability to travel up to 50%.
  • Registered Nurse (RN) licensed in a state with current hospice operations, with the ability to obtain licensure in additional states as needed.

Nice To Haves

  • Experience in survey preparation and response, plan-of-correction development, and regulatory remediation.
  • Experience supporting interdisciplinary clinical teams in multi-site hospice operations.
  • Proficiency in Microsoft Office applications.

Responsibilities

  • Partner with hospice leadership to ensure high-quality interdisciplinary clinical practice across agencies.
  • Provide expert guidance on hospice standards of care, documentation practices, symptom management frameworks, and clinical escalation pathways.
  • Conduct clinical assessments, case reviews, and targeted interventions to improve patient outcomes, reduce live discharges, and support appropriate hospice utilization.
  • Offer direct clinical consultation to Executive Directors, Clinical Managers, and interdisciplinary team members on complex cases.
  • Monitor and evaluate compliance with Medicare Conditions of Participation, state regulations, and internal policies.
  • Conduct clinical audits and documentation reviews to identify compliance gaps and opportunities for improvement.
  • Lead mock survey activities, collaborate on plan-of-correction development, and support agencies through remediation.
  • Stay informed on CMS and state regulatory updates and ensure timely dissemination to agency leadership.
  • Serve as a subject matter expert and super user for the hospice EMR (e.g., Homecare Homebase).
  • Provide clinical workflow guidance during system configuration, build, and optimization activities.
  • Support training efforts during new system rollouts, upgrades, or workflow changes in collaboration with the Hospice Education team.
  • Collaborate with Quality and Compliance teams on QAPI initiatives, clinical performance measures, and targeted improvement plans.
  • Analyze clinical quality metrics (e.g., HIS, CAHPS, live discharge rates, visit patterns) and assist agencies in developing action plans.
  • Guide agencies in implementing performance improvement initiatives and multidisciplinary improvement projects (MDIs).
  • Support bereavement, volunteer, and interdisciplinary programs to ensure alignment with quality standards.
  • Assist with clinical staffing optimization, workload balancing, and operational planning for multiple hospice locations.
  • Contribute to the development and maintenance of the Hospice Clinical & Operating Playbook.
  • Document and update clinical SOPs, workflows, and best practices to maintain consistent application across sites.
  • Support change management efforts for new clinical initiatives, standards, and care delivery models.
  • Work with interdisciplinary teams across the enterprise to align clinical operations with the organization’s mission, regulatory expectations, and growth strategies.
  • Ensure consistent application of policies and procedures across markets.
  • Participate in meetings, project teams, and educational programs as required.

  • Provide clinical coaching, support, and capability development for agency and clinical leaders.
  • Deliver functional guidance to hospice leadership on clinical practice improvements, regulatory alignment, and performance expectations.
  • Support training, change adoption, implementation of new clinical standards, and performance improvement initiatives across agencies.
  • Contribute to development and maintenance of scalable, sustainable clinical operating models.
  • Assist with hiring, evaluation, and development input for agency clinical teams as requested.
