The Clinical Social Worker is responsible for managing assigned patient caseloads across inpatient medicine and surgery, the Addiction Recovery Program, or the emergency department. This role involves providing bio-psychosocial assessments and interventions for patients and their families, addressing acute medical needs and psychosocial factors impacting care. As an integral member of the interdisciplinary team, the social worker identifies high-risk psychosocial factors, helps staff understand their influence on patient care, and intervenes through evaluation, emotional supportive therapy, resource provision, and team collaboration to overcome barriers to care and facilitate discharge planning. The social worker must be knowledgeable in mandated reporting, cultural and age-specific impacts, homelessness, substance use disorders, psychiatric illness, and available community resources.
