About The Position

Legacy Health is seeking Clinical Social Workers committed to holistic patient-centered care, providing adult and pediatric care in diverse settings, and advocating for vulnerable populations. Clinical Social Workers evaluate psychosocial and mental health issues, manage transitions and crises, and coordinate internal and external services. They also assess intervention strategies for behavioral and addiction issues, end-of-life care, and therapeutic decision-making. The role involves advocating for patients, their surrogates, family members, caregivers, and healthcare providers. The organization is considering candidates with an Independent Clinical Social Work Associate License (LSWAIC) for a Clinical Social Worker I position or those with an Independent Clinical Social Worker License (LICSW) for a Clinical Social Worker II position. Legacy Medical Group comprises over 50 primary care clinics and specialty practices with more than 800 care providers, offering access to a wide range of specialists within the Legacy system.

Requirements

  • Master of Social Work degree from an accredited School of Social Work required.
  • Licensed Social Worker (LSWAIC, working toward LICSW) required in WA (for Clinical Social Worker I).
  • Associates in WA may begin work with a "pending" associate license and may work for a maximum of 120 days while awaiting licensure approval (for Clinical Social Worker I).
  • CPR certification required: American Heart Association (AHA) issued BLS Provider certification.
  • 2 years relevant experience in care management, mental health and/or addictions services, medical social work, or social service organization (for Clinical Social Worker II).
  • Licensed Independent Clinical Social Worker (LICSW) required in WA (for Clinical Social Worker II).
  • Broad knowledge of the principles of social work and the ability to apply them with individuals, families and/or groups integrating knowledge of normal development and the aging process, changes related to illness, and the needs of patients with acute, chronic, and terminal illness in assessment and intervention with attention to cultural and literacy implications.
  • Knowledge of medical, mental health and substance use diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology.
  • Strong organizational, communication and collaboration skills, including an ability to effectively triage patient and family needs, function as member of an interdisciplinary team, identification and collaboration with community stakeholders or resources and written documentation.
  • Knowledge about social determinants of health, principles of diversity/equity/inclusion and culturally responsive practice with populations who have experience historical marginalization.
  • Demonstrates ability to locate, utilize and advocate for patient/family access to hospital/ community resources.
  • Exhibits knowledge of legal/regulatory issues and an ability to be a resource to all team members regarding such issues.
  • Ability to navigate electronic medical records and effectively utilize basic computer programs.
  • Knowledge of legal, regulatory and compliance requirements and an ability to be a resource to all team members regarding such issues and knowledge of current health care environment.

Nice To Haves

  • 1 year of relevant experience in care management, mental health and/or addictions services, medical social work, or social service organization (for Clinical Social Worker I).
  • 3 years' relevant experience (for Clinical Social Worker II).

Responsibilities

  • Provides social work services in a variety of adult and pediatric settings across the health care continuum with a focus on holistic patient-centered care and attention to the needs of vulnerable populations.
  • Works both independently and collaboratively as part of an interdisciplinary team.
  • Key services include (but are not limited to) comprehensive evaluation of psychosocial and mental/behavioral health status, legal and safety considerations, substance use or addiction, chronic illness, or end of life issues with the use of brief counseling, transition planning, crisis management and planning and coordination with internal and community stakeholders.
  • Social work intervention may be with individuals, family members or groups.
  • Utilize a strengths-based approach throughout the care continuum with the aim to enhance the capacity of people to address their own needs.
