Requirements

  • Demonstrate strong relationship-building skills with internal and external stakeholders
  • Exhibit strong clinical skills and knowledge with the ability to collaborate with clinical team members to develop effective treatment plans
  • Aptitude to learn and understand funding sources and criteria
  • Ability to articulate and communicate with funding sources the need for inpatient level of care and the intensive rehab being provided
  • Dependable and accountable to funding deadlines
  • Willingness to understand, implement, and comply with current State and Federal Regulations
  • High school diploma or GED ( required)
  • Annual flu vaccine is required during flu season
  • A drug screen and criminal background check are required.
  • Must not have any condition or limitation that could pose a direct threat to self or others unless such threat can be eliminated by a reasonable accommodation.
  • Must be able to pass a fitness assessment designated for this job title and perform all physical activities related to the position.
  • Regular attendance in your assigned role is an essential job function.

Nice To Haves

  • At least one year of experience in resident/patient assessment (preferred)
  • Candidate would hold a degree in either nursing (RN or LPN) or other clinical/therapy degrees such as Speech-Language Pathology, Physical Therapy, Occupational Therapy, Counseling, or Social Work (preferred)

Responsibilities

  • Collaborate and maintain relationships with physicians, therapists, nursing team, referral sources, social workers, clinical team, and funding sources to develop and implement individualized therapy programs for our residents
  • Facilitate meetings with family and provide education about our program
  • Display understanding of the discharge process and how to support families as they transition their loved one back home
  • Provide support to clinical and residential teams as necessary
  • Efficiently drive clinical programs and engage in healthy intellectual conflict in order to provide excellent outcomes
