About The Position

HopeHealth is seeking an experienced administrative support individual for its affiliate HopeHealth Hospice and Palliative Care. This position will provide timely, responsive, customer-focused service to all patients, families, staff, students, vendors and visitors to HopeHealth. Acts as first line communication for all customers (both internal and external), providing a positive, warm, calm and competent image to all customers when responding to customer needs. The Hospice Service Coordinator provides administrative support and assistance to HopeHealth in coordination and support of patient care delivery. This is a 40-hour, full-time day position, 8:00am-4:30pm; with an every other weekend requirement. This position will be primarily based in our Providence, RI location, but may need to travel to another facility in Wakefield, RI or Attleboro, MA if the need arises.

Requirements

  • High school diploma plus additional specialized training. Associate degree or graduate of a secretarial school program preferred. Two years related experience and/or training; or equivalent combination of education and experience.
  • Valid driver's license with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Must be able to use computer proficiently. Significant experience with data base programing at the input and report running end is required. Knowledge of Excel (or similar spreadsheet) and Microsoft Word (or similar word-processing software) is required.

Responsibilities

  • Provides first line customer service by responding to customer calls. Provides information to callers or routes calls to appropriate personnel, takes messages or relates information in a confidential, accurate, timely and professional manner.
  • The Coordinator is a liaison between the patient, the care giving staff, other internal and external departments and the billing department, supporting all three with comprehensive chart management and communications between parties and ensure seamless care.
  • Orders, cancels, and changes durable medical equipment (DME) supplies as required by agency staff. Communicates with agency members regarding the status of DME supplies ordered. Keeps log of DME supplies ordered for patients within the Interdisciplinary Group (IDG) module, DME tab in myUnity.
  • Runs reports from data systems such as IDG, CTI, and Face to Face (F2F), physicians and recertification’s as requested or scheduled by agency clinical manager.
  • Daily morning distribution of aide’s schedules and agency report of staffing, including Hospice Medical Doctor (HMD), patient admissions, deaths, transfers and/or revocations/discharges.
  • Provides all needed documentation for IDG meetings. Data enters edits generated from IDG meeting discussions. Participates as directed in IDG meetings. Logs and stores attendance sheets and IDG packet for future use.
  • Communicates all pertinent patient status information to appropriate team members in timely manner. Types, files, photocopies, faxes as needed to support the team and site activities. Docusign forms sent and received, forwarded to necessary parties and retain electronic storage of copies and forward all signed copies to HIM.
  • Works collaboratively with all Agency Service Coordinators to ensure department needs and requirements are achieved.
  • Assists with the scheduling of Hospice Aides to assure services, staff and schedules are accurate and current. Keeps Clinical Manager or designee informed about Hospice Aide coverage and staffing issues.
  • Consistently apply the HopeHealth values in all aspects of work, complying with established HopeHealth mission, policies, procedures and objectives in quality assurance, safety, environment, infection control and HIPPA compliance.
