Responsibilities

Serves as clinical resource for patient care and oversight for assigned care teams, assists with patient assignments and determination of patient acuity levels.
Responsible for assuring the development, implementation, and updates to the plan of care, recommending additional services to meet patient needs and achieve positive outcomes.
Facilitates case reviews and/or case conference meetings for coordination of patient care.
Participates in discharge planning processes and coordinates the referral process to ensure care across the continuum.
Makes recommendations for efficient and effective utilization of resources to achieve positive outcomes above the national norm by implementing clinical protocols during the admission visit as dictated by patient’s diagnosis.
Monitors Infection Control Prevention activities and works with clinicians to perform Infection Control Surveillance.
Maintains competency requirements regarding regulatory requirements, provides input and assures compliance with departmental policies and procedures, scope of care, safety, environmental and infection control standards to achieve positive patient outcomes.
