About The Position

Provide specialized care to specific patient populations, ensuring optimal care and coordination between providers, patients, and healthcare teams. Act as a liaison for patient testing, scheduling, and referrals while assisting in patient education and the implementation of care plans.

Requirements

  • Current unencumbered licensure with the WV Board of Licensed Practical Nurses, or the appropriate state board where services will be provided, as a Licensed Practical Nurse.
  • State criminal background check and Federal (if applicable), as required for regulated areas.
  • Certification in Basic Life Support within 30 days of hire.
  • Five (5) years' experience as an LPN.
  • Uses competent clinical practice and critical thinking skills to efficiently deliver patient care while maximizing efficient use of resources.
  • Ability to be flexible in response to changes in work volume, staff and scheduling changes.
  • Knowledge of and appropriate application of the nursing processes.
  • Ability to communicate clearly with patients, families, physicians, and other employees in both verbal and written form.
  • Possesses the interpersonal skills to positively and effectively communicate, negotiate, and resolve conflict.

Responsibilities

  • Provide hands-on assistance to patients in accordance with the care plan, including preparing or administering medications, performing basic tests, assisting with procedures, and obtaining specimens under the supervision of an RN or physician.
  • Act as a point of contact for patients, coordinating appointments, testing, and procedures as needed. Work with scheduling departments to ensure timely care.
  • Support care plan implementation by assisting in the implementation of care plans developed by RNs and physicians, ensuring appropriate follow-up and delivery of care as per the established plan.
  • Assist with obtaining prior authorizations for patient services, working with healthcare providers and insurance companies as needed.
  • Provide basic education to patients about their conditions and treatments under the direction of an RN or healthcare provider.
  • Accurately documents patient care activities, assessments, and progress according to departmental guidelines and under the supervision of an RN or physician.
  • Work with RNs, physicians, and other healthcare providers to ensure the continuity and quality of patient care. Assist in administrative tasks as necessary to support patient care delivery.
