We are looking for people who share our values of integrity, excellence, and compassion. We are Better Together! Our Hospice is looking for a Clinical Referral Coordinator. This position reports to the Intake Manager and will work Monday - Friday, 8 am – 4:30 pm and every 3rd weekend (flex days will be provided the weeks you work weekends). This position shall be responsible for coordinating the referral to admission process including: managing the intake for referrals, explaining the Hospice program, collecting and reviewing documentation for eligibility of the Hospice admission criteria, coordinating with the clinical team and scheduling admissions. Responsibilities utilize knowledge of computer systems, office procedures, regulations, and Our Hospice of South Central Indiana goals and objectives.

Requirements

  • Practical Nursing Degree preferred; or equivalent combination of education and experience.
  • Valid Indiana LPN license
  • One plus years’ experience in a fast paced office setting required
  • Valid Indiana Driver’s License.
  • Ability to work in a fast-paced environment and manage multiple priorities simultaneously.
  • Strong oral and written communication skills; excellent listening skills; excellent interpersonal and motivational skills; and the ability to interpret instructions furnished in a variety of formats, i.e. written, oral, diagram, etc.
  • Ability to effectively present information to and respond to questions from patients/families and the general public.
  • Strong attention to detail.
  • Flexibility to be a strong team player.
  • Ability to work in a fast-paced environment and manage multiple priorities simultaneously.
  • Ability to summarize data for reports.
  • High level of interpersonal skills to handle sensitive and confidential situations.
  • Must be able to effectively utilize Microsoft Word and Excel.

Responsibilities

  • Makes the initial contact on referrals as required, explaining Hospice services and assists the Business Development Representatives in evaluating the patient’s ability to meet Hospice’s admission criteria.
  • Responds timely to all referrals, fostering relationships of trust, responsiveness, and open communication with all referring customers including but not limited to hospital case managers, physicians, facility staff, patients and families.
  • Conducts chart review of patients to help determine eligibility and DME needs.
  • Collaborates with the Intake manager on all clinical concerns and appropriately escalating clinically complicated cases and those with high acuity DME needs.
  • Requests records and information from referrals sources, enters required information into system, completes documentation requirements and orders DME to support Business Development Representatives.
  • Backup to the Business Development Representatives for family information sessions.
  • Promotes the Hospice benefit and provides education to all potential referring customers including but not limited to case managers, extended care facilities, physicians and physician office staff, and community groups.
  • Manages daily and weekly activity reporting to business development team, clinical team and leadership to promote communication and teamwork.
  • Manages accurate and timely input of new referrals into the system.
  • Collaborates with the Intake Manager to coordinate the admission schedule and availability. Coordinates the transfer of the patient to contracted facilities or home as indicated.
  • Facilitates communication and works effectively between Hospice staff, patients/families/caregivers, physicians, vendors, and other professionals as appropriate.
  • Participates on assigned committees and/or staff meetings.
  • Participates in the provision of educational and community relations activities to the community and other health care professionals as requested.
