About The Position

Make a meaningful impact in your community-join Acentra Health as a Clinical Evaluator (PASRR) in Hutchinson, Kansas. In this role, you will conduct PASRR Level II Pre-Admission Screening and Resident Review assessments to help determine appropriate placements and next levels of care for individuals with complex needs. As a Clinical Evaluator, you will conduct behavioral health assessments for the geriatric population as well as individuals with Intellectual Disabilities (ID), Serious Mental Illness (SMI), or Related Conditions (RC) across nursing homes, hospitals, and community-based settings. These evaluations play a vital role in identifying the appropriate next level of care and supporting improved long-term health outcomes. This PRN hybrid role offers both flexibility and purpose combining remote work from your home office with local travel. Acentra Health provides reimbursement for travel-related expenses associated with this role, including mileage paid at the federal reimbursement rate and covered parking. We provide all necessary technology and equipment needed for your home office setup, including a company-issued laptop for use both at home and in the field.

Requirements

  • Must possess an active, unrestricted Kansas license in one of the following Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Marriage and Family Therapist (LMFT), Licensed Independent Clinical Social Worker (LICSW), Licensed Clinical Professional Counselor (LCPC), Licensed Clinical Psychologist (LCP), Registered Nurse (RN), or another qualified Licensed Mental Health Professional (LMHP).
  • Graduation from an accredited college with a relevant degree program and minimum of 2 years of experience assessing individuals with mental health or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive impairments, and dementia.
  • Proven ability to assess and analyze psychological, emotional, behavioral, and physical functioning to support accurate and comprehensive clinical evaluations.
  • Familiarity with mental health, intellectual and developmental disabilities (IDD), geriatrics, and other conditions commonly requiring long-term care services and supports.
  • Strong communication skills, with the ability to interact effectively and with empathy with individuals and groups through verbal, written, and electronic communication.

Nice To Haves

  • Strong understanding of public mental health systems and medical programs, including public-sector, state-funded, and community-based services, as well as Level of Care guidelines and related standards of practice.
  • Knowledge of psychiatric disorders as outlined in the current Diagnostic and Statistical Manual (DSM) used in Kansas.
  • Detail-oriented with excellent organizational skills and the ability to prioritize and complete tasks efficiently within established timelines.
  • Previous experience working with the geriatric population.
  • Proficiency in computer-based software and tools, including Microsoft Office Suite (Word, Excel, Outlook, Teams).

Responsibilities

  • Responsible for conducting PASRR (Preadmission Screening and Resident Review) assessments and evaluations, and for developing informed level-of-care recommendations across diverse settings-including nursing homes, hospitals, and community environments. This includes determining the appropriate next level of care for individuals who may require long-term care placement, while effectively assessing each individual's unique needs.
  • Adhere to the state's best standards of mental health practices, intellectual and developmental disabilities, geriatrics, and other conditions requiring nursing care services and supports.
  • Ensure compliance with contractual and regulatory standards while developing level of care recommendations
  • Utilize independent judgment, deep clinical expertise, exceptional communication skills, and problem-solving abilities to navigate complex situations, ensuring the best outcomes for members, providers, and clients.
  • Foster effective collaboration with a diverse range of healthcare professionals, patients, families, and support networks while monitoring personal metrics to ensure compliance with turnaround time requirements..
  • Stay informed on current literature, regulations, and research pertaining to long-term care, mental illness, medical conditions, and geriatrics.
  • Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities.
  • Execute tasks with attention to detail, ensuring adherence to established workflows to maintain high standards of quality and operational efficiency.
  • Understand, and comply with all corporate policies, including those related to HIPAA and its Privacy and Security Rules.
